Training Forms

The following forms need to be filled out every year. Each grade has different forms that need to be filled out. The grade level is based on the 2010/2011 school calendar. For example if your child is going to be a junior for the 2010/2011 school year then print off the junior pages. Please print off and sign all paperwork that is associated with your athlete's classification. All forms need to be turned into the field house training room. If Coach Martinez are not there then please leave the forms by the door.
